Summary Care Record (SCR)

The NHS in England has introduced the Summary Care Record (SCR), an electronic health record that can be accessed when you need urgent treatment from somebody other than your own GP. 

Your Summary Care Record will be available only to authorised healthcare staff and they will need to ask your permission before they look at it. Those who can look at your Summary Care Record need to have an authorised Smartcard with a chip and unique secure password.

Your Summary Care Records contain key information about the medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had in the past.

As a patient you have a choice:

Yes I would like a Summary Care Record - You do not need to do anything and a summary Care Record will be created for you.

No I do not want a Summary Care Record - If you do not want a record, you need to complete a Summary Care Opt Out Form and return it to us. You should do this even if you have already completed a form at your previous practice.

You can choose not to have a Summary Care Record and you can change your mind at any time by informing us.

If you do nothing it is assumed that you are happy for Summary Care Record to be created for you. Children under 16 will automatically have a Summary Care Record created for them unless their parent or guardian chooses to opt them out. If you are the parent or guardian of a child under 16 and feel that they are old enough to understand, then you should make this information available to them

Summary Care Record with Additional Information

You can also choose to share your Summary Care Record with Additional Information. This includes your long term health conditions, relevant medical history, health care preferences, personal preferances and immunisations.

You GP Practice may recognise that having additional information in your SCR will be of benefit to you and may suggest this change. Additional information will only be included after discussion with your GP Practice and only if you give your permission.
