New Patient Registration
If you wish to register with our practice please speak to the receptionist at your nearest surgery who will be happy to assist you.
You will also be given a new patient registration pack - this will contain all the details you need to register with the surgery and documents to sign you up to register for online services.
You will be asked to complete a registration form plus a questionnaire and provide one form of personal identification plus proof of address.
All patients are registered under a named partner of the practice but are able to see any doctor as their preferred practitioner when the need arises and if there are available appointments.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out, print and bring with your registration form. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
